If you qualify, you will gain access to the Connection portal where you will find out more information about the study and sign the informed consent forms. Here you will also fill out your baseline survey and provide proof of prescription.

Once the baseline period is complete, you will have follow-up surveys to complete on the Connection portal at 1 month, 3 months, and 6 months.

Yes! If all study-related activities are completed, participants may receive a total of $100. How it works is that you are eligible to receive $25 after completing the baseline tasks and then you are eligible to receive an additional $25 per completed touch point at the end of month 1, month 3, and month 6.
Participation is completely voluntary and can be ended at any time.
