Effective date: September 18, 2026

Provider: 1nHealth, Inc., 424 E Central Blvd #129, Orlando, FL 32801

  1. Eligibility. You may use the app only if you have applied to a study supported by 1nHealth, or you are the parent or caregiver who submitted a study application on behalf of a child, and you sign in with the email address given in that application. You must be at least 16 years old.
  2. What the app is for. The app shows information about your study application and lets you confirm visits and request contact. It does not provide medical advice, diagnosis or treatment. For medical questions, contact your study team or a healthcare professional. In an emergency, call your local emergency number (911 in the United States).
  3. Accuracy. The information shown comes from your study team’s records and may lag behind real events. If something looks wrong, contact your study team.
  4. Your account and responsibilities. Keep your device secure. Do not share your password or the sign-in code emailed to you. Sign out on a device you no longer use or that you share with someone else. You are responsible for activity on your account until you sign out or tell us it has been compromised.
  5. Electronic communications. By using the app you agree to receive sign-in codes and account notices by email, and notifications on your device if you allow them.
  6. Acceptable use. Do not attempt to access anyone else’s information, interfere with the service, or use the app for anything other than its purpose.
  7. Availability. We may change, suspend or withdraw the app or any feature at any time, including when your study application ends.
  8. Ending your use. You can stop using the app at any time by signing out and deleting it, or by deleting your account from the Profile tab. We may end your access if these terms are broken or when your study participation ends.
  9. App stores. The app is distributed through app stores such as the Apple App Store and Google Play, and their terms also apply to your download and use of it.
  10. Intellectual property. The app and its content belong to 1nHealth, Inc. or its licensors. You receive a personal, non-transferable licence to use it for its purpose.
  11. No warranties. The app is provided as it is, with all faults. We make no promises, express or implied, about its accuracy, reliability or completeness. What you see comes from your study team’s records, so if a date or a detail matters, confirm it with your study team.
  12. Limitation of liability. Under no circumstance will 1nHealth, or any of its officers, directors or employees, be liable for any loss, damage, injury, liability or expense arising out of or connected with your use of the app, whether the claim is made in contract, in tort or otherwise, and including indirect and special damages.
  13. Indemnity. You agree to indemnify 1nHealth and its affiliates against all actions, claims, damages and expenses arising from your use of the app or from your breach of these terms.
  14. Governing law. These terms are governed by and construed under the laws of the state in which 1nHealth operates. You unconditionally submit to the non-exclusive jurisdiction of the courts located in the state of Florida for the resolution of any disputes.
  15. Changes to these terms. We will post any changes here and update the effective date.
