By answering these questions, you agree to provide your information to 1nHealth. Your information will only be used for the purposes of this study. The form asks about your health, age, and contact information to see if you pre-qualify for the study.

Contact

We also ask for your contact information, which will only be used to contact you for the purposes of this study. If you pre-qualify, you will be called by a study representative, and if you qualify, you will be contacted by a research site. By agreeing to this privacy policy, you are agreeing to be contacted by study representatives by phone and email for screening for this study. Your information will not be shared with Vertex Pharmeceuticals in any way that could identify you. In the event that you do not qualify, you will not be contacted. You will also have the option to delete the information after you complete the pre-screener by emailing [email protected]. If you choose to apply to another study through 1nHealth, Vertex is not responsible for your data.
